The Norwood scale, and what it can't tell you

The Norwood scale is the most-searched thing in hair measurement and the most misread. It is a set of pictures of shapes. It has no time axis, no density, and no arithmetic.

Where it came from

James Hamilton introduced a classification of patterned hair loss in men in 1951. O'Tar Norwood revised and expanded it in 1975 into the numbered seven-stage system still in use, adding the Type A variants. That is why you will see it called the Norwood scale, the Hamilton scale, or the Hamilton–Norwood scale — they are the same lineage. The seven stages, in plain terms

Each stage is a described shape. Nothing more.

VIIThe most advanced form: a narrow horseshoe band at the sides and back.

There is also a set of Type A variants, in which the front border moves back evenly across the whole scalp without a separate patch developing at the crown. They are numbered to line up roughly with the standard stages.

The four things it can't tell you

These are not complaints invented for this page. They are the limitations documented against the scale itself, set out in Wikipedia's summary of the scale.

1. It cannot tell you how fast anything is moving

This is the big one, and it is the thing everyone actually wants to know. The scale does not predict the rate of progression. Someone at stage III may sit there for decades, or may not. A stage is a position, and two people standing in the same position arrived at different speeds.

Speed is the information that matters, and the only way to get it is to measure the same head more than once, under conditions you controlled. One stage number, assigned once, contains no speed at all.

2. It ignores the characteristics of the hair itself

The system does not account for strand calibre, colour, curl pattern, or density per follicular unit — all of which change how the same outline looks. Two heads matched at the same stage can look substantially different, because the scale is tracing a border and not weighing what is inside it. This is the difference between an outline and what density actually means.

3. It is assigned by eye, and eyes disagree

Studies of inter-rater reliability have found only moderate agreement among dermatologists classifying the same patients; the boundaries between adjacent stages are not always clear-cut. If trained clinicians looking at the same person land on different stages, then two photos of you taken an hour apart landing on different stages is not a mystery — it is the expected behaviour of an ordinal visual scale.

4. The numbers are not quantities

The stages are ordered, but the gaps between them are not equal and not measured. Stage IV is not "twice" stage II. Going from III to IV is not the same amount of change as going from V to VI. You cannot average stages, subtract them, or plot them as if the axis were linear. A great deal of anxiety comes from doing arithmetic on a scale that does not support arithmetic.

What it is genuinely good for

None of the above makes the scale useless. It is good at exactly one job: giving two people a shared vocabulary for a shape, so that a conversation about a pattern is about the same pattern. That is a real contribution and it is why the scale survived fifty years.

It is a noun, not a verb. It names where something is. It says nothing about where it is going.

It was built for one pattern, in men

The scale was designed for male pattern hair loss and does not apply to female pattern hair loss, which typically presents as diffuse thinning across the central scalp rather than recession at the front. Other classifications — the Ludwig and Sinclair scales — exist for that. Applying a Norwood number where the pattern does not match the scale's design produces a number with no meaning behind it.

What Crown reports instead

Crown does not assign you a Norwood stage. It reports a hairline stage on a 1–6 appearance scale as one of seven appearance metrics, alongside three separately scored zones — hairline, temples and crown — each with its own 0–100 reading.

The reason for splitting it up is the second limitation above. A single whole-head label collapses three regions that move independently into one bucket, and the bucket cannot tell you which region moved. Three numbers and a date can. That is the entire argument for scoring the zones separately.

What Norwood stage am I?

No one can tell you that from a description, and a single stage is a less useful answer than it feels like. A stage records the shape of a border at one moment. It carries no information about how fast anything is changing, which is what most people are actually asking.
